Proof of vaccinations is required before a dog goes on a Nature Hike (whether Solo or Group)

The Emergency Veterinarian Authorization Form is strongly recommended for all clients, but is not required before beginning any service with me. We will both fill it out and each have a copy; your copy will be kept on file with your vet.

Which vaccines does this documentation prove?

Rabies

Distemper, including Leptospirosis (usually denoted as DHLPP = Distemper, Hepatitis, Leptospirosis, Parainfluenza and Parvovirus)

Bordetella (kennel cough)

Canine Influenza Vaccine (It is denoted as CIV, which is a separate vaccination from Parainfluenza)

Are there any other documents of proof I should expect to be emailed or texted to me rather than uploaded in this form?
